As part of the Patriot Hospice Team, you are part of specially trained professionals who act as a bridge to care and resources on a person’s end-of-life journey.

Palliative care is specialized medical care for people with serious illnesses. It focuses on providing relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family.  Palliative care is provided by specially trained team who work together to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. 

Physician Services brings healthcare to our patient’s home, whether that is a residential home, assisted living or independent living facility.  We provide all services that your traditional primary care office provides but all from the comfort of home.  We also work with community partners to provide in home x-rays, EKGs, labs, etc.  We accept most insurances and their no additional fees to bring services to the home.
